Insurance Information
Our office will work closely with you on your insurance coverage. Be sure to provide us with all the necessary information to accurately process your insurance forms. Most plans do not cover 100% of treatment costs. This does not mean that the procedures are not needed. It simply means that your insurance company may not cover the treatment. Our office will instruct your insurance company to send benefit payments directly to you. We expect you to be responsible for your account with us. In this way the insurance company is directly accountable to you, as it should be.
